Goals and target groups
What are the goals of the research focus PARTICIPATE?
The research focus aims to initiate participatory research processes based on the needs identified by the network partners on the topic of transition situations. The network partners are representatives of patients, affected people and relatives as well as representatives of health-related and nursing care in the region. The research focus generates projects that are highly relevant to the people affected and to practice, with the aim of:
- to promote and maintain the participation and autonomy of older people in particular when the need for care occurs or increases
- to enable related and affected people to make jointly sustainable decisions
- to enable health professionals to define and exercise their professional role in transition situations and to support transition processes in an interprofessional, cooperative and evidence-based manner to identify and
- to strengthen suitable conditions with regard to the framework conditions for participatory and interprofessional decisions in health and social care organizations.
The research focus is also intended to create awareness that certain challenges in transition situations can be met preventively by preparing decisions in a participatory and interprofessional manner.
